Who to contact about your claim
Claims Assistance and Re-Employment Team: 801-526-4400
This guide is independent. Nobody here can open your claim, change it, or see what is on it, and nothing sent to this site reaches Workforce Services. The numbers, Live Chat, and the fraud form below are the Department's own.
Help with your own claim
A Utah claim is managed in a "my Unemployment" account, reached by signing in at jobs.utah.gov, and Live Chat sits inside that account. The Claims Assistance and Re-Employment Team takes the calls, on a separate number for each part of the state: 801-526-4400 covers Salt Lake and South Davis Counties, and the rest are in the table at which number to call about what, along with the overpayment line, the line for moving a Re-Employment Services and Eligibility Assessment appointment, and Relay Utah for callers who are deaf, hard of hearing, or speech-impaired.
Calls and Live Chat both run Monday to Thursday, 7:00 a.m. to 3:00 p.m., and Friday, 7:00 a.m. to 12:00 p.m. Sending something in writing instead? How to send paper has the fax numbers and the mailing address.
Live Chat sits behind the sign-in, so when the sign-in itself is the problem the phone is what is left. Employment Centers hold no unemployment insurance information and cannot help with a claim, and what to do if you cannot get online sets out what they do offer instead.
Questions about an appeal go to the Appeals Unit rather than to the CARE Team. Its number, its post office box, and the deadline that runs the whole thing are on denials, appeals, and overpayments.
Reporting fraud, or a claim filed in your name
Utah takes a fraud report online, through the Department's fraud form, or by phone on 801-526-4400, option 2. That is also where to start if benefits you never drew turn up on a tax form in your name, and a tax form for a claim you never filed has the correction to ask for. What fraud costs a claimant who commits it is set out under fraud and what it costs.
Asking the Department directly
The button above opens the Department's own Unemployment Insurance contact page, where it keeps the numbers, the fax lines, and the post office boxes for each part of the division.
For anything that is not a call or a chat, a request, question, comment, problem, or suggestion, the Department also takes it through its own electronic Feedback Form. For paperwork about your own claim, mail it to Unemployment Insurance, P.O. Box 45266, Salt Lake City, UT 84145-0266; that address is separate from the Department's address for an employer's question about unemployment insurance taxes, which is not for a claimant's paperwork.
Use Live Chat or the CARE Team for anything with a deadline on it, because a week that has to be claimed, or a decision that has to be appealed, will not wait for the post.
